Health and Social Care Partnerships

The Health and Social Care Partnerships Team works with integrated care systems across England, Wales and Northern Ireland to identify service needs through co-production.

We work with systems to identify local need and help them to create services that deliver on quality and value for money.

We co-produced this guide with 20 commissioners from across the UK and people living with dementia. It provides commissioners with the tools, tips and knowledge to involve people with lived experience in their work.
